Applicants may also mail their form to<\/p>\n<address>WisDOT<\/address>\n<address>Special Plates Unit &#8211; DIS ID<\/address>\n<address>PO Box 7306<\/address>\n<address>Madison, WI 53707-7306<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Permanent placards are free. Temporary placards cost $6. Checks or money orders for that amount can be made payable to Registration Fee Trust, though applications made at local DMV\u00a0customer service centers are subject to an additional counter service fee.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards are valid for 4 years. Temporary placards are valid for 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewal of placards<\/h5>\n<\/div>\n<p>Permanent placards must be renewed every 4 years and require recertification of disability.<\/p>\n<p>Those with temporary placards whose\u00a0disability extends to more than 6 months and who wish to retain their placards must complete and submit a new application, which will be valid for another six months.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Lost or stolen permit must be reported to the address listed above, and replacements can be obtained by completing the applicable form.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>The placard must be hung from the rear view windshield of the vehicle when parked in a designated parking space.\u00a0Operating a motor vehicle when a permit is hanging from the rear view mirror is\u00a0a safety hazard and creates an obstruction\u00a0to a driver\u2019s clear view through the front\u00a0windshield. Violators may be fined up to $100.<\/p>\n<div id=\"misc\">\n<h5>Miscellaneous<\/h5>\n<\/div>\n<p>Placard holders are advised keep a copy of their application when submitting it to Wisconsin DOT.
