The application must then be submitted \u00a0in person at any DMV Regional Office or mailed to<\/p>\n<address>Division of Motor Vehicles<\/address>\n<address>Mobility Impaired Placard and Plates<\/address>\n<address>PO Box 17010<\/address>\n<address>Charleston, WV 25317<\/address>\n<address>\u00a0<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>All placards are free.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards are valid for 5 years; temporary placards are valid for 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing placards<\/h5>\n<\/div>\n<p>Permanent placard holders will receive a renewal reminder at the address initially provided to the DMV Regional Office. Recertification of disability will be required.<\/p>\n<p>Temporary placards cannot be renewed. If the temporary disability persists after the expiration date, disabled individuals need to submit a new application.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>If a parking placard or special license plate is lost, stolen, mutilated or destroyed, a replacement may be requested at <span style=\"font-size: 13px; line-height: 19px;\">any DMV Regional O\u001ece or by mail at the address listed above.
