The form can then be submitted at the local DMV.<\/p>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Placards are free.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards are valid for 2 years upon issuance. Temporary placards are issued for a period not exceeding 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing placards<\/h5>\n<\/div>\n<p>The DMV will mail permanent placard holders a renewal notice, which must be signed and mailed back to the DMV. Those who lose their notice or do not receive one, can visit their local DMV or call (801) 297-7780 or 1-800-368-8824 to renew the placard over the phone.<\/p>\n<p>Temporary placard holders whose disability extends beyond the 6-month limit need to reapply by following the same procedure at initial application.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>A replacement placard can be obtained by completing the Disabled Person and Physician Disability Certification (Form TC-842).<\/p>\n<ul>\n<ul>\n<li>You <b>DO NOT<\/b> need a new physician&#8217;s certification.<\/li>\n<li>You <b>DO<\/b> need the number of your lost or stolen disability placard or plate.<\/li>\n<li>Submit the completed application form and payment (if applicable) <b>in person<\/b> to your local <a href=\"https:\/\/www.dmv.org\/ut-utah\/dmv-office-finder.php\">Utah DMV branch office<\/a> or <b>by mail<\/b> to:<\/li>\n<\/ul>\n<\/ul>\n<p>Division of Motor Vehicles<\/p>\n<p>Mail and Correspondence<\/p>\n<p>P.O.
