The form is available at <a href=\"https:\/\/www.dmv.ri.gov\/contact\/#2\" target=\"_blank\" rel=\"nofollow\">any DMV office<\/a> or <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Rhode Island online.pdf\">online<\/a>. Once \u00a0must then be mailed to<\/p>\n<address>Division of Motor Vehicles<\/address>\n<address>Disability Parking Placards Office<\/address>\n<address>600 New London Avenue<\/address>\n<address>Cranston, RI 02920-3024<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>There is no fee required to obtain a handicap parking placard.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>A temporary placard is issued for a minimum of 2 months and a maximum of 12 months. Placards for long-term conditions are issued for 1-3 years. Permanent placards are valid for more than 3 years.<\/p>\n<div id=\"renewals\">\n<h5>Renewing placards<\/h5>\n<\/div>\n<p>Placards can be renewed using the same application as was used to request a new permit.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Forms for lost, stolen, or worn placards are available at <a href=\"https:\/\/www.dmv.ri.gov\/contact\/#2\" target=\"_blank\" rel=\"nofollow\">any DMV office<\/a> or online.
