According to ORS 801.387, &#8220;person with a disability&#8221; means someone who<\/p>\n<ul>\n<li>has severely limited mobility because of paralysis or the loss of use of some or all legs or arms;<\/li>\n<li>is affected by loss of vision or substantial loss of visual acuity or visual field beyond correction; or<\/li>\n<li>who has any other disability that prevents the person from walking without the use of an assistive device or that causes\u00a0him or her to be unable to walk more than 200 feet, including but not necessarily limited to chronic heart condition, emphysema, arthritis, rheumatism, ulcerative colitis, or related chronic bowel disorder.<\/li>\n<\/ul>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>Applicants must<\/p>\n<ul>\n<li><span style=\"line-height: 12.997159004211426px;\">have a valid Oregon Driver License or an Identification Card to obtain a renewable, temporary duplicate (travel) or wheelchair permit; and<\/span><\/li>\n<li><span style=\"line-height: 12.997159004211426px;\">complete the Parking Permit for an Individual with a Disability application (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Oregon Form 735-265.pdf\">Form 735-265<\/a>), which must be signed by a licensed physician.<\/span><\/li>\n<\/ul>\n<p>Those who qualify for a &#8220;no photo&#8221; parking identification card as certified by a physician, as well those seeking family or program disabled person parking permit, must mail the application to the address below. All others may drop it off at <a href=\"https:\/\/www.oregon.gov\/ODOT\/DMV\/pages\/offices\/index.aspx\" target=\"_blank rel=\" rel=\"noopener\">a DMV office<\/a> or mail it to<\/p>\n<address>DMV Driver Issuance Unit<\/address>\n<address>1905 Lana Ave NE<\/address>\n<address>Salem, OR 97314<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>There is no fee for a disabled person parking permit.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>A renewable permit is valid until the expiration of the holder&#8217;s driver license or ID card.\u00a0A temporary permit is valid for the length of time the physician deems necessary, not to exceed 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing permits<\/h5>\n<\/div>\n<p>Applicants must submit a new certificate of disability each time they wish to renew their permits.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>To replace a lost, stolen, or damaged permit, <a href=\"https:\/\/www.oregondmv.com\/SSL\/contactdmv\/driver.htm\" target=\"_blank\" rel=\"nofollow noopener\">contact the DMV<\/a>.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>When parked, hang the disabled person parking permit from the rearview mirror. If the vehicle does not have a rearview mirror, place the permit on the dashboard.
