It must be accompanied by a prescription from a health care provider that states the date, name of the person with the disability, and the expected duration of the disabling condition. The health care provider must sign the prescription and state that it is for a disability placard.<\/p>\n<p>Applicants may find the form <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Ohio online.pdf\">online<\/a>, at <a href=\"https:\/\/bmv.ohio.gov\/county_lst.stm\" target=\"_blank\" rel=\"nofollow\">any local deputy registrar license agency<\/a>, or by calling (614) 752-7800 or (800) 589-TAGS. Once it&#8217;s been completed, applicants may drop it and the required processing fee of $3.50 off at <a href=\"https:\/\/bmv.ohio.gov\/county_lst.stm\" target=\"_blank\" rel=\"nofollow&quot;\">any local\u00a0deputy registrar license agency<\/a> or mail it to<\/p>\n<address>Ohio\u00a0Bureau of Motor Vehicles<\/address>\n<address>Attn.: Registration<\/address>\n<address>Remittance Processing Section<\/address>\n<address>P.O. Box 16521<\/address>\n<address>Columbus, OH 43216-6521<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>There is a $3.50 processing fee for all placards.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Temporary (red in color) and permanent (blue in color) placards expire on the date prescribed by the health care provider.<\/p>\n<div id=\"renewals\">\n<h5>Renewing placards<\/h5>\n<\/div>\n<p>Temporary placards cannot be renewed. Renewal applications are mailed 60 days prior to the expiration date for permanent placard holder (individuals and organizations).<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Applicants whose placards are lost or stolen may replace their placards by completing <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Ohio Form BMV 4826.pdf\">Form BMV 4826<\/a>.
