Commission of the Blind.<\/li>\n<\/ul>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>Applicants must complete the application form (SP-41), which can then be dropped of at the local Motor Vehicle Commission (MVC) agency or mailed to<\/p>\n<address>NJ Motor Vehicle Commission Special Plate Unit<\/address>\n<address>PO Box 015<\/address>\n<address>Trenton, NJ 08666-0015<\/address>\n<p>&nbsp;<\/p>\n<p>Applications for temporary placards are available at any local police station or <a href=\"https:\/\/images.myparkingpermit.com\/doc\/New Jersey online.pdf\">online<\/a>. A qualified medical practitioner must certify that the applicant qualifies for a temporary placard. The completed form, along with a check or money order made payable to the Motor Vehicle Commission in the amount of $4.00, must be submitted in person to a municipal chief of police.<\/p>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Permanent placards are free.\u00a0Temporary placards are issued for a fee of $4.00.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placard holders must be medically recertified every three years. Temporary placards are valid for 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing placards<\/h5>\n<\/div>\n<p>Permanent placard holders must be medically recertified every three years. The <a href=\"https:\/\/images.myparkingpermit.com\/doc\/New Jersey recertification application.pdf\">recertification application<\/a> may be dropped off at any local motor vehicle agency or mailed to<\/p>\n<address>NJ Motor Vehicle Commission Database Correction Unit<\/address>\n<address>PO Box 141<\/address>\n<address>Trenton, NJ 08666-0141<\/address>\n<p>&nbsp;<\/p>\n<p>Permanent placards are renewed every 3 years, and renewal notices are mailed to the placard holders. Temporary placards are allowed a one-time six-month renewal.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p><span style=\"line-height: 13px;\">Damaged, stolen, or lost placards may be replaced by visiting any MVC agency. If the applicant&#8217;s placard is<\/span><\/p>\n<ul>\n<li><span style=\"line-height: 13px;\">damaged, he or she must bring the damaged placard and his or her original &#8220;Person with a Disability Identification Card&#8221;;<\/span><\/li>\n<li>lost, he or she must bring his or her original\u00a0&#8220;Person with a Disability Identification Card&#8221;;<\/li>\n<li>stolen, he or she must bring a copy of the police report, as well as\u00a0his or her original\u00a0&#8220;Person with a Disability Identification Card.&#8221;<\/li>\n<\/ul>\n<p>If both the placard and\u00a0&#8220;Person with a Disability Identification Card&#8221; are lost or stolen but not yet expired, it may be possible to\u00a0reissue a new placard and identification card. The applicant must have\u00a0a driver license or other form of identification, though, and a\u00a0notarized statement or police report attesting to the loss or theft of the identification card and placard.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>Placards must be\u00a0displayed on the rear view mirror of the vehicle whenever it&#8217;s parked in a designated wheelchair\u00a0symbol parking space.
