A certifiable medical condition may include, but is not limited to,<\/p>\n<ul>\n<li>severe visual or physical impairment;<\/li>\n<li>severe respiratory problems;<\/li>\n<li>a Class III or Class IV cardiac condition; or<\/li>\n<li>loss of all or substantially all the use of one or more limbs.<\/li>\n<\/ul>\n<p>Facilities such as retirement homes, hospitals,\u00a0schools and places of a similar type involved in the transportation of handicapped individuals may also receive\u00a0disability\u00a0parking permits.<\/p>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>Applicants must contact their Nebraska-licensed medical professional (physician, physician&#8217;s assistant, or nurse practitioner) to request a permit. That individual must then request a new or renewal permit through the <a href=\"https:\/\/www.clickdmv.ne.gov\/\" target=\"_blank\" rel=\"nofollow\">Online Handicap Parking Permit Service<\/a>.<\/p>\n<p>Applicants working with medical professionals who aren&#8217;t licensed in Nebraska can use the <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Nebraska Application for Handicapped Parking Permit.pdf\">Application for Handicapped Parking Permit<\/a>; its medical certification section must be completed by the medical professional.<\/p>\n<p>The completed and signed application must be submitted, along\u00a0with a photocopy of acceptable proof of identification, to the<\/p>\n<address>Nebraska Department of Motor Vehicles<\/address>\n<address>Driver and Vehicle Records Division<\/address>\n<address>Attn: Handicapped Parking Permits<\/address>\n<address>PO Box 94789<\/address>\n<address>Lincoln, NE 68509-4789<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Handicapped parking permits are free.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of permits<\/h5>\n<\/div>\n<p>Permanent permits (blue) are valid for 6 years and expire on the last day of the applicant&#8217;s birthday month in the sixth year of the permit&#8217;s issuance.<\/p>\n<p>Temporary placards (red) are valid for up to 6 months. Their expiration dates are determined by the expected date of recovery indicated in the medical certification section of the <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Nebraska Handicapped Parking Permit Application.pdf\">Handicapped Parking Permit Application<\/a>.<\/p>\n<div id=\"renewals\">\n<h5>Renewing permits<\/h5>\n<\/div>\n<p>To renew a permit, applicants follow the same procedure as for the initial issuance. Applications can be submitted 180\u00a0days prior to expiration. Temporary permits can only be renewed once for an additional maximum 6-month period.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>A lost or stolen placard can be replaced by completing and signing the same application as was completed during the request for a new permit. The check box specifying &#8220;replacement&#8221; must be checked. Applicants do not need to have their disability recertified by a medical professional. The completed application must be submitted with a photocopy of acceptable proof of identification and mailed to the local DMV.<\/p>\n<p>Permit holders are limited to two replacements.
