Persons with this condition alone are only eligible for temporary placards.<\/li>\n<\/ul>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>Applicants must submit an Application for Disability Plates\/Placards (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Maine Form PS-18.pdf\">Form PS-18<\/a>). Its top section must completed by the applicant; its bottom section must be completed by a physician,\u00a0physician&#8217;s assistant, nurse practitioner, or a registered nurse. The application may be processed at any Bureau of Motor Vehicles branch or mailed or faxed to:<\/p>\n<address>Bureau of Motor Vehicles\u00a0<\/address>\n<address>Disability Clerk<\/address>\n<address>29 State House Station<\/address>\n<address>Augusta, ME 04333-0029<\/address>\n<address>Fax: (207) 624-9204<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Placards are free.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>A permanent placard is valid for 4 years, while temporary placards are valid for up to 6 months.<\/p>\n<h5>Renewing placards<\/h5>\n<p>Permanent placards must be renewed every 4 years. Applicants must complete \u00a0the top portion of <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Maine Form PS-18.pdf\">Form\u00a0PS-18<\/a>. Because recertification isn&#8217;t required, the bottom portion can be left blank when renewing a placard.<\/p>\n<p>Renewal of temporary placards require the completion of both the top and bottom portions of <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Maine Form PS-18.pdf\">Form PS-18<\/a>.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Contact the Bureau of Motor Vehicles or visit thenearest Motor Vehicle Branch office for a replacement. The applicant&#8217;s paperwork<a href=\"https:\/\/www.maine.gov\/sos\/bmv\/registration\/TTY%20changes\/PS-18%20DX%20form%20Rev%206-2012.pdf\" target=\"\" rel=\"nofollow\">\u00a0<\/a>must be on file and current with BMV. The expiration date of the new replacement placard will be the same as the old placard.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>The placard must be hung from the rearview mirror when the vehicle is stationary and parked in the space designated for persons with disabilities.
