Who’s eligible?<\/h5>\n<\/div>\n

A person is eligible if he or she has a severe visual, audio, or physical impairment, including partial paralysis, lower limb amputation, chronic heart condition, emphysema, arthritis, rheumatism, or other debilitating condition that limits or impairs his or her personal mobility or ability to walk.<\/p>\n

Any agency or organization that transports persons with a disability as a part of the service provided by that agency or organization is also eligible.<\/p>\n

How to obtain a permit<\/h5>\n<\/div>\n

Applicants must complete an Application for Disabled Person’s Special Parking Permit,<\/a> which must\u00a0signed by a licensed physician, as well as signed and notarized by the applicant. The signed form must be submitted to the local County Clerk’s office<\/a>.<\/p>\n

Agencies or organizations applying for a placard must also submit the form and include<\/p>\n
