Those applying for a temporary placard can submit a completed form to any Secretary of State facility or mail it to the following address. Those applying for a permanent placard must mail their completed form to:<\/p>\n<address>Secretary of State<\/address>\n<address>Persons with Disabilities License Plates\/Placard Unit<\/address>\n<address>501 S. Second St., Rm. 541<\/address>\n<address>Springfield, IL 62756<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Placards are issued free of charge.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards (blue) are valid for 4 years. Temporary placards (bright red) are issued for a maximum of 6 months. Organizational placards, which are available to groups that transport disabled people, are valid for 4 years from their date of issue or until 2018, whichever comes sooner.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your placard<\/h5>\n<\/div>\n<p>Renewing a placard is the same as the original application process. Applicants must fill out Form <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Illinois%20Persons%20with%20Disabilities%20Certification%20for%20Parking%20Placard%20License%20Plates%20form.pdf\">VSD 62.23<\/a> and mail it to the address above.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>To replace a lost, damaged, damaged, or stolen placard, applicants must complete form <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Illinois VSD 415.9.pdf\">VSD 415.9<\/a> and pay a $10.00 replacement fee (unless the applicant is eligible for the Circuit Breaker program, in which case it is free).
