Person with a disability means someone who<\/p>\n<ul>\n<li>cannot walk 200 ft or more without using \u00a0a walker, cane, crutch, braces, wheelchair, a prosthetic device, or another person;<\/li>\n<li>cannot walk 200 ft or more without discomfort or difficulty due to a neurological, orthopedic, respiratory, cardiac impairment, an arthritic disorder, blindness, or the absence or loss of function of a limb;<\/li>\n<li>has a permanent disability where the physician certifies that there is no expectation for a fundamental change in the condition in the future.<\/li>\n<\/ul>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>The applicant must complete the Disability License Plate And Placard application (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Idaho Form ITD 3392.pdf\">Form ITD 3392<\/a>) and mail it to<\/p>\n<address>Idaho Transportation Department<\/address>\n<address>Vehicle Services &#8211; Special Plates<\/address>\n<address>PO Box 7129<\/address>\n<address>Boise, ID 83707-1129<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Both permanent and temporary placards are issued free of charge.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Because permanent placards are issued in perpetuity, there is no expiration date. Temporary placards (red) are valid for 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your placard<\/h5>\n<\/div>\n<p>Only temporary placards are subject to renewal. To renew them, the applicant must resubmit\u00a0the Disability License Plate And Placard application (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Idaho Form ITD 3392.pdf\">Form ITD 3392<\/a>), which includes recertification by a physician. The completed form must be mailed to the address shown above.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Lost, stolen, or damaged cards are reissued at no charge.
