The form must include certification of disability by the applicant&#8217;s doctor.<\/p>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Placards for individuals with permanent disability, temporary disability, and disabled drivers in specially equipped vehicles are free.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>A permanent parking placard is issued for 4 years after which it must be renewed.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your permit\/placard<\/h5>\n<\/div>\n<p>A <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Georgia Disabled Persons Parking Affidavit.pdf\">Disabled Person&#8217;s Parking Affidavit<\/a> must be submitted every 4 years for renewal after a permanent disabled parking placard expires.
