If the assistive device significantly restores his\/her ability to walk to the extent that he\/she can walk without severe limitation, he\/she will no longer be eligible for the exemption parking permit.<\/li>\n<\/ul>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>The applicant must complete\u00a0<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Florida Form HSMV 83039.pdf\">Form HSMV 83039<\/a>\u00a0and submit it to the <a href=\"https:\/\/www.flhsmv.gov\/offices\/\" rel=\"nofollow\">local county tax collector&#8217;s<\/a> license plate agency.<\/p>\n<p>Applicants who have previously completed Form HSMV 83039 and had it signed by a certifying authority within the last 12 months can apply via mail or in person at the local tax collector office or license plate agency. Applicants who choose the latter option should bring a copy of their Florida driver&#8217;s license or ID card.<\/p>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>There is no fee for a permanent disability parking placard. There is a $15.00 fee for temporary placards.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards are issued for a period up to 4 years and expire on June 30th. Temporary placards are issued up to 6 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your permit\/placard<\/h5>\n<\/div>\n<p>Individuals wishing to renew their placards must submit a completed\u00a0certificate of disability (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/Florida Form HSMV 83039.pdf\">Form HSMV 83039<\/a>) signed by the certifying authority within the previous 12 months. It can be submitted to the tax collector office or tag agency.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Lost or stolen placards can be replaced by submitting Form HSMV 83039 signed by the applicant&#8217;s certifying authority within the last 12 months.\u00a0The completed form can be submitted to the tax collector office or tag agency.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>The\u00a0permit must hang on the rear view mirror of any vehicle used to transport the disabled person(s). The permit number must be visible from the front of the vehicle.<\/p>\n<div id=\"misc\">\n<h5>Miscellaneous<\/h5>\n<\/div>\n<p>All parking permits must have a valid decal.
