The placard may be used in any vehicle the placard holder operates or in which he or she is transported.<\/p>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>Applicants must complete and sign a Certificate of Eligibility (<a href=\"https:\/\/images.myparkingpermit.com\/doc\/New Mexico MVD-10383.pdf\">MVD-10383<\/a>). \u00a0Photocopies will not be accepted.\u00a0Completed forms may be taken to any New Mexico Motor Vehicle Division field office for processing or mailed to the following address:<\/p>\n<address>New Mexico Motor Vehicle Division<\/address>\n<address>1100 South St. Francis Drive<\/address>\n<address>Santa Fe, NM 87505<\/address>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>Disability parking placards are issued at no cost.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<div>\n<p>Permanent placards expire 4 years from their date of issuance. Temporary placards are valid for a maximum period of 12 months.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your permit\/placard<\/h5>\n<\/div>\n<p>No reminder notice will be mailed to the placard holder prior to a\u00a0placard&#8217;s\u00a0expiration date.<\/p>\n<p>Holders of permanent placards must renew their placards before the expiration date (4 years from the date of issuance). No further medical information will need to be furnished, but <a href=\"https:\/\/images.myparkingpermit.com\/doc\/New Mexico Form MVD-10383.pdf\">Form MVD-10383<\/a> must again be completed and submitted to any field office or mailed to the address above. Put another way, Section D of the form will not need to be refilled, only Sections A and E.<\/p>\n<p>Holders of temporary placards may renew their placards by following the same process required for the original issuance of the placard.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>Those wishing to replace a damaged placard may simply bring it to a field office, where it will be reissued at no cost.<\/p>\n<p>Those wishing to replace a lost or stolen placard may call the Motor Vehicle Division at (888) 683-4636, where a customer service representative can offer assistance. (Be prepared to provide a driver&#8217;s license or identification number.)<\/p>\n<p>Sections A and E of Form MVD-10383 also allow placard holders to request replacement placards. The form can be brought in to any field office or mailed to the address above.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>When the vehicle is parked, the placard must be hung from the rear view mirror with the identification number and expiration date visible from the front of the vehicle. If there is no rear view mirror, the placard should be placed on the dashboard with the identification number and expiration date visible.<\/p>\n<p>The placard is to be displayed <strong><em>only<\/em> <\/strong>when the vehicle is parked, <em><strong>not<\/strong> <\/em>when it is in operation.
