Part A of the form must be completed by the applicant; Part B must be completed by a licensed medical professional. The form can be emailed to dmv.hpapp@ct.gov or faxed to (860) 263-5556. A $5.00 fee is to be paid for temporary parking permit.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>A permanent parking permit is valid for the same period as the license or identification card, which is typically 4 to 6 years. All permits expire the last day of the month of expiration.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your permit\/placard<\/h5>\n<\/div>\n<p><a href=\"https:\/\/images.myparkingpermit.com\/doc\/Connecticut Form B-225.pdf\">Form B-225<\/a> needs to be completed and signed, specifying the renewal requirement by ticking the appropriate check box.
