The form must be signed by either the applicant or a representative. A licensed Colorado medical professional or a commissioned medical officer (CMO) must sign the form.<\/p>\n<div id=\"fee\">\n<h5>Fee<\/h5>\n<\/div>\n<p>There is no fee for placards.<\/p>\n<div id=\"valid\">\n<h5>Validity\/expiration of placards\/permits<\/h5>\n<\/div>\n<p>Permanent placards are valid for up to 3 years, while temporary placards are valid for up to 90 days. Temporary placards may only be renewed once.<\/p>\n<div id=\"renewals\">\n<h5>Renewing your permit\/placard<\/h5>\n<\/div>\n<p>Permanent placard holders need to recertify their disability status every 3 years by filling out <a href=\"https:\/\/images.myparkingpermit.com\/doc\/Colorado DR 2219.pdf\">DR 2219<\/a>.<\/p>\n<div id=\"lost\">\n<h5>Lost placard\/replacement<\/h5>\n<\/div>\n<p>If the placard has been lost, stolen, mutilated, or otherwise damaged and the replacement placard is being requested prior to the expiration date, the damaged or mutilated placards should be surrendered to the authorized agent for replacement.
