Who’s eligible?

An eligible candidate for a placard is someone who

  • uses portable oxygen;
  • cannot use one or both legs;
  • cannot walk 200 ft. at length without stopping;
  • cannot walk without using an assisting device, such as braces, crutches, prosthetic device, wheelchair, another person, etc.;
  • suffers from an arthritic, neurological, or orthopedic condition that severely limits the ability to walk; or
  • has a Class III or Class IV cardiac condition as defined by the standards set by the American Heart Association.
How to obtain a permit

Applicants must complete and sign the Application for Disabled Parking Identification (Form 861) and have an Alaska licensed physician fill out and sign the appropriate section of the form, which must be subsequently submitted to the DMV. The application may be submitted in person or by mail.

Fee

There is no fee for placards.

Validity/expiration of placards/permits

Permanent placards are valid for 5 years. Temporary placards are valid for a maximum of 6 months.

Renewing your permit/placard

Upon request by the applicant, a second permit or renewal of an expiring/expired permit may be issued. You may request a new application for disabled parking identification (a signature is not required when submitted by the disabled individual) or print the ALVIN record as proof that the individual has/had a valid permit. Identification is required to ensure that the permit is being issued to the person named on the application or ALVIN record.

Lost placard / replacement

A replacement permit can be issued when a permit has been lost, destroyed, or stolen.

(i) On a print-out of the permit record from the ALVIN files, the applicant must write the reason that a new permit is being requested. This should be written directly on the print-out.

(ii) The applicant must sign the statement.

(iii) Immediately enter a message on the ALVIN record using the “U” (update) function to let others know what happened to the previously issued permit.

Where to hang the placard/permit

The placard must be displayed on the rearview mirror when the vehicle is stationary but removed when the vehicle is in motion.
